Please fill out our New Patient Form. Once completed, please contact one of our team members at (435)652-1897 to schedule an appointment with one of our providers. Please note that we do ask that all new patients pay a $50 deposit to hold the appointment. This deposit will act as a credit on your account to be applied towards any copay, deductible, or personal pay amount, or our late cancellation/missed appointment fee.

They all accept most major insurances: i.e: Select Health, BCBS, Cigna, PEHP, EMI, UHC, Medicare, Aetna *We also offer cash-pay rates.

Once an appointment is scheduled, you will be expected to pay for it unless you provide 24 hours advance notice of cancellation.  There is a $50.00 missed appointment or late cancellation fee that is to be paid before you are able to reschedule.  Medication Management cancellation fee is $50. Therapy cancellation fee is $120. It is important to note that insurance companies and other third party reimbursement sources do not provide reimbursement for late cancelled sessions or missed appointments.  You will be responsible for payment of missed appointments or late cancellations.
